DATES AND TIMES:

*Prior to trying out for a PEVA Girls Travel Team, you must attend one of the Form Frenzy sessions listed below.

Saturday 10/11 @9:30-10:30am

Sunday 10/12 @9:30-10:30am

Thursday 10/16 @7-8pm

Friday 10/17 @7-8pm

Saturday 10/18 @8-9am and 11am-12pm 

*There will be limited staff members on Saturday due to on court duties. Please provide enough time to complete all tasks.*

WHAT TO BRING:

Athletes should attend Form Frenzy as uniform samples will be available to try on for correct sizing. 

Paperwork-wise, you'll need:

1. KRVA/USAV Tryout Membership $15 (Under Membership Types, click on the link "Keystone Region - Junior Tryout Pass")

2. 2025-2026 USAV Medical Release Form (this form does not need to be notarized)

3. COVID 19 Waiver (this can also be completed in the Active system during the registration process)

4. Pittsburgh Elite Waiver and Authorization (this can also be completed in the Active system during the registration process)

 

*You can print these forms and fill them out prior to Form Frenzy, or we will have hard copies available which can be filled out during Form Frenzy*

FAQs:

What is Form Frenzy Early Check-in?

Athletes and their parents/guardians complete the travel tryout check-in process early in the week, before tryout day. 

 

Why would Pittsburgh Elite offer an Early Check-in?

We've offered Form Frenzy for the past three years (prior to COVID) and found that it provides a much better tryout experience for athletes, their families, and our staff.  Early check-in allows us to take time to answer any questions you might have or to offer assistance with registration without trying to also balance an on-court tryout simultaneously.  Our coaches love to catch up with returning families and Form Frenzy is just the place to do that!

 

What happens at Form Frenzy?

Athletes and their parent/guardian complete the tryout check-in process.  You'll turn in your paperwork, athletes select uniform sizes, and you leave with a numbered shirt to wear for tryouts and information regarding the travel program.  Director Ashley Green, and Pittsburgh Elite coaches are on hand to answer any questions, learn about what the Pittsburgh Elite experience is like and help with the process.

 

Is there an advantage to coming to Form Frenzy?

Yes!  Aside from completing the check-in process in a more relaxed atmosphere, our coaching staff will know to expect you at tryouts.  If you haven't completed the online registration, you can do so at Form Frenzy for the reduced price.  Once Form Frenzy concludes, registration prices increase.

 

What if I can't make Form Frenzy?

Please contact Club Director, ASHLEY GREEN.  Alternatively, you can arrive prior to your scheduled tryout session to complete check-in.  Please allow yourself enough time to turn in paperwork, try on uniform samples, and receive your numbered tryout shirt.
